Email SGO@alaska.gov a letter of interest signed by an authorized representative, attesting that the organization meets and will continue to meet the federal requirements, plus a copy of its IRS 501(c)(3) determination letter. There is no form and no fee, and applications are reviewed on a rolling basis.
